All applicants for Patient Transport Officer or Ambulance Transport Attendant positions in Victoria must provide a Statement of Attainment that includes the unit HLTOUT007 – Transport Non-Emergency Patients Under Operational Conditions.

Please upload your 5-year Driver History Record (Demerit Point History and Full Driving Record), issued within the last 3 months by your state’s road authority. For Victorian applicants, this is a 3-page VicRoads document that includes: A verification page confirming your identity. Your current demerit point status. A full list of penalties and offences. Residency Status

Work Rights Verification

NPT is required to confirm that all employees have the legal right to work in Australia. If you progress to the final stages of recruitment, NPT will register you with Make Sure, our external verification provider. Make Sure will contact you directly by email and request any documents needed to complete the residency or VEVO check. You will upload your documents securely to Make Sure, not to NPT. Make Sure will advise NPT of the outcome once the check is complete. NPT does not collect or store passports, birth certificates, citizenship certificates, or visa documents.

National Police Check

The Police Check must indicate the position type e.g. Patient Transport Officer, Ambulance Attendant, Registered Nurse or as a minimum state Patient Transport.

Referee Requirements

If you’re shortlisted after interview, we’ll ask for the contact details of two professional referees who have supervised your work. You may include these now in your résumé if you wish.
(Referee details are not required at this stage.)

Step 5: Immunisation and Health

To protect patients and staff — and to meet Australian Infection Control and WHS requirements — NPT requires evidence of mandatory vaccinations before employment. Please upload your Medicare Immunisation History Statement (and serology where requested). If you need advice, speak with your GP.

! NPT will be unable to progress any application for employment until evidence of the required vaccines has been provided.

EXAMPLE OF A SEROLOGY (BLOOD TEST) REPORT

If your Immunisation History Statement does not show the required number of vaccine doses, you must provide a serology report to confirm your immunity through blood test results.

Please note: Your Medicare Immunisation History Statement cannot be accepted as evidence of serology. Serology refers to a blood test that confirms your immunity status and must be provided separately.

Please complete all sections.  The pre-employment health questionnaire does not determine your suitability for employment.  This assessment is only one component of the overall recruitment process.  It is not the only factor determining whether you are successful in gaining employment.  

Please note that you may also be required to undertake a pre-employment functional assessment.  If this is required, you will be advised by e-mail.
